Change of Address Form
Anytime you move, please update your address with the Blackfeet Tribe Enrollment Office by sending us a notarized form. Otherwise, any information we send out to you, such as Per Capita checks will be mailed to the address we have on file.
Release of Confidential Information Form
If you would like for anyone other than yourself to pick up your enrollment documents such as Tribal ID's, Certificate of Indian Blood, Copy of Birth Certificate, or Per Capita Check, Please fill out this form. This form must be notarized or our office will not be able to release this information.

Document Prerequisites
Marriage and Divorce Name Change
A copy of the original legal marriage license or divorce court papers must be submitted to the Enrollment Department before any changes can be made to your name.
Adoption and Name Change
An original birth certificate with the name change is required. Until this document is submitted to the Enrollment Department, no changes can be made to your name. This is not required for marriage or divorce cases.
Custody Changes for Minor Children
A legal court order and address change form is required and must be submitted to the Enrollment Department before any changes can be made regarding Parental Custody or Legal Guardianship of a minor child.
Certificates of Indian Blood
Certificates of Indian Blood are available free of cost. Per Blackfeet Tribal Resolution 110.91. This official document approves Certification of Blackfeet Membership and Indian Blood Quantum, and is signed by the Chairman of the Blackfeet Tribe and the Enrollment Technician.
Blackfeet Tribal ID's
Blackfeet Enrollment and Decendant Identification Cards are available at the Enrollment Department during business hours only. You must be in person and show proof of identity as with any other local, state, or federal agency. More Information
Enrollment Guidelines
The Blackfeet Tribe has strict enrollment guidelines that must be met to become an enrolled member. Membership into the tribe is based on our constitution and by-laws as well as our procedures that govern our enrollment

When applying for enrollment into the Blackfeet Tribe an application is required with an original birth certificate. Applications can be picked up at the Enrollment Department or mailed to you upon request. November 15th of each year is the deadline for new applicants attempting to make eligibility for the annual December Per Capita payment.
Ineligible applicants will be notified by mail with the return of all original documents.
If the applicant qualifies as a descendant of the tribe, a descendant form will be mailed to you automatically. The descendant form only verifies that you have a direct relationship to an enrolled Blackfeet tribal member, such as your parents, grandparents, and so on.
